Plain English

Although officially unaffiliated with Indiana University, the event takes place on the Indiana University campus and is categorized as a “Program Involving Children”. As such, the University requires us to obtain consent for three documents: 1) Photo Release 2) Liability Release and 3) Emergency Contact.

Photo Release – Signing this document allows us to photograph the participant during conference settings and use the photos without restriction in promotional contexts.
Liability Release – Signing this document absolves the program and the University of liability in the event that certain actions cause harm to the participant.
Emergency Contact – Completing this document allows us to contact someone in the event that an emergency involving the participant occurs.
